Logging in as a representative

Parents can access their child’s patient portal. With authorization, you log in using your own DigiD. On this page, you’ll find information that matches different age groups. You can also read about authorization as a legal representative.

Parent

You need authorization. You can request this from the doctor’s assistant at the department where your child is being treated.

You will need:

  • your child’s ID

  • your own ID

  • your child’s consent if they are older than 12

Is your child 16 or older? Then your child can authorize someone directly through DigiD.

From age 16, any previous authorization automatically expires. Through DigiD Authorization, your child can give online consent to someone they trust, such as one of the parents. That person can then log in to the child’s patient portal using their own DigiD and manage matters on the child’s behalf.

Your child can request the authorization at machtigen.digid.nl. It can also be withdrawn at any time on that same website.

If your child is legally incapable of making decisions, the authorization can be extended through the doctor’s assistant until the age of 18.

Legal representative

You need authorization. You can request this from the doctor’s assistant at the department where the child is being treated.

You will need:

  • the child’s ID

  • your own ID

  • a court order stating you are the child’s guardian or mentor

  • the child’s consent if they are older than 12
